The East Lancashire Alliance is currently looking to recruit a Full-time Nurse Associate on behalf of the Rossendale PCN. Please note:

This role will be based at various locations across Rossendale, working as part of the TNA / NA visiting service for Rossendale PCN. Own car and Full Driving License is essential. Interviews will be taking place on 17.01.2025. Main duties of the job

The Nurse Associate will: Be responsible for delivering evidence-based, high-quality, patient-centred, compassionate care every time. Be an accountable professional. Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual. Have the ability to work without direct supervision at times. Have the capacity to deliver care independently in line with the individual’s defined plan of care within the parameters of practice of the Nurse Associate role. Be able to access clinical and care advice when needed. The responsibility of the postholder is to participate in: The planning and delivery of Enhanced Health in Care Homes model of proactive care that is centred on the needs of individual residents, their families, and care home staff. Chronic Disease Reviews with housebound patients. Clinical Sessions within a practice setting. About us

The Post holder will: Maintain a professional portfolio, identifying role and skill developments in line with the NMC Code of professional practice. Keep up to date with current national developments in nursing practice and policy. Ensure NMC revalidation requirements are met. Contribute to PCN wide/departmental policies, procedures, and guidelines. Maintain professional working relationships with staff and respect the equality and diversity of each person they come into contact with in the course of their business. Deliver high-quality, compassionate care under the direction of a Community Link Nurse (or other registered care professional dependent on setting) with a focus on promoting health and independence. Have proficient attitudes and behaviours compatible with NHS Values. Carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of lead nurse or other registered care professionals dependent on setting. Knowledge, training, and experience Work within clearly defined accountability frameworks and boundaries of limitation to provide the best possible care in partnership with colleagues, patients, and their carers. Contribute towards developing a culture of learning and innovation, developing high-quality learning environments. Communicate effectively across a wide range of individuals, the public, and health and social care professionals. Communicate information received from the individuals, carers, and members of the team appropriately. Ensure all patient-related information is treated appropriately, always adhering to the principles of confidentiality. Ensure clear, concise, accurate, and legible records are made and that all communication is maintained in relation to care delivery adhering to local and national guidance. Exercise personal responsibility and work independently within defined parameters of practice, taking the initiative in a variety of situations and performing a range of clinical and care skills consistent with the role, responsibilities, and professional values of a Nursing Associate. Exercise judgment in assessing patient condition, comfort, and wellbeing using analysis of a range of possible factors. Develop, improve, and maintain a professional portfolio, identifying role and skill developments in line with the NMC Code of professional practice. Keep up to date with current national developments in nursing practice and policy. Contribute to PCN wide/departmental policies, procedures, and guidelines. Maintain a professional appearance in line with the Rossendale PCN dress code. Carry out specific delegated clinical and care tasks and responsibilities to a high standard and competency, under the direction and supervision of Community Link Nurses or other registered care professionals dependent on setting. Clinical responsibilities (The following list is indicative as tasks and responsibilities will vary depending on the care setting the Nurse Associate is working in) The Nurse Associate will: Provide and monitor care. Develop understanding of all elements of the nursing process and be able to assist the lead nurse in the ongoing assessment, planning, management, and evaluation of care. Support individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence through awareness raising and care navigation to other professionals as appropriate. Perform and record clinical observations including blood pressure, temperature, respirations, pulse, and NEWS. Accurately record nutritional and fluid intake. Ensure the privacy, dignity, and safety of individuals is always maintained. Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate. Report back and share information with the registered nurses on the condition, behaviour, activity, and responses of individuals. Recognise issues relating to safeguarding adults and report any problems or raise concerns to the appropriate registered care professional. Assist in the implementation of appropriate action to meet the specific physical, emotional, and psychological, social, cultural, and spiritual needs of individuals and carers. Assist in the delivery of care as prescribed by the lead nurse. Develop understanding of caring for individuals with conditions for example dementia, mental illness, learning disabilities. Develop skills in relation to coaching/teaching individuals/carers/other staff. Assist with the implementation and monitoring of clinical standards and outcomes. Develop a working knowledge of other providers’ resources and referral systems to ensure individuals' needs are met, within parameters of practice. Engage in reflective practice including management of self and reflection on own reactions, asking questions and reflecting on answers given. Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures. Demonstrate good understanding of the Mental Health Act / Deprivation of Liberty Standards (DOLs) and applies principles to everyday practice seeking advice/guidance from the Community Link Nurse or registered care professional as required. Qualifications

Registered Nurse Associate on the NMC register. Care Certificate. Educated to a minimum level 5 foundation degree specific to nursing associate practice. GCSE level 4-9 in Maths and English or equivalent. Experience

Previous experience of working within a health / care setting. Understanding of the importance of the promotion of health and wellbeing (Making Every Contact Count). Knowledge and Skills

Clean Driving Licence and own transport. Understanding of the scope of the role of the Nurse Associate in context of the team and the organisation, and how the role may contribute to service development. Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team. Insight into how to evaluate own strengths and development needs, seeking advice where appropriate. Evidence of time management skills and ability to prioritise. IT skills appropriate to the role. Ability to communicate with members of the public and health and care providers. Courteous, respectful, and helpful at all times. Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact. Ability to work on own initiative, be well motivated and motivate others. Ability to take part in reflective practice and clinical supervision activities. Able to demonstrate resilience, managing personal emotions and stress. Knowledge of when to seek advice and refer to a registered care professional. Ability to engage and complete preceptorship. Understanding of evidence-based practice.
