An exciting opportunity has arisen at Benchill Medical Practice in Wythenshawe, Manchester to recruit a Paramedic to work as part of a multidisciplinary team to serve our 10,500 patients. We belong to Wythenshawe Primary Care Network which consists of 7 practices serving 60,000 patients. This exciting opportunity is to work directly in one practice to support patients from varying backgrounds and patient demographics. The role is pivotal to improving the quality of care and operational efficiencies, so it requires motivation and passion to deliver excellent service. The role will provide remote and face-to-face assessment of health and clinical needs for patients presenting to primary care. The post holder will triage, clinically assess, examine and develop a clinical impression, differential diagnosis, and final diagnosis as appropriate to inform their preferred treatment plan for patients across placement settings. Main duties of the job

Develop and maintain a specialist level of knowledge across a range of clinical procedures. This will typically involve specialist knowledge and skills in triage, history taking, structured physical examination skills, problem-solving, and clinical decision-making to develop a clinical impression/differential/final diagnosis for patients. Work alongside a nominated GP in providing a first point of contact for patients presenting with undifferentiated, undiagnosed problems across a variety of clinical or non-clinical settings according to patients' needs (e.g., at the patient's home). Evaluate information from clinical examinations and/or triage to initiate appropriate treatment and/or referral. This includes appropriate signposting of patients to plan care for patients. Plan provision of care for patients as appropriate, e.g., onward referral to clinical services, self-care advice, and prevention advice. Consider holistic aspects and work in accordance with wider guidelines, protocols, policies, and patient care pathways. Work in partnership and liaise with multi-disciplinary teams within primary, urgent, and community secondary care, to promote integrated approaches to patient care and seamless patient referral into appropriate healthcare services and pathways. Ensure safe handover of care within and outside GP and NWAS as appropriate. Communicate sensitive and confidential information concerning patients requiring empathy, persuasion, and reassurance. The postholder will typically work under the remote supervision and direction of a GP or Advanced Clinical Practitioner, developing their clinical skills and knowledge to work more independently and benefit patients across the entire healthcare system. Your role is an essential part of our evolving dynamic and forward-thinking practice, working to provide enhanced care experience to patients in line with a view to improving health inequalities. Experience

Consolidated experience of clinical practice Ambulance and or primary care settings, including assessing patients with minor illnesses and injuries. Understanding of patient management within primary and urgent care settings. Experience of clinically supervising peers or junior colleagues in practice. Working knowledge of 999 primary and secondary triage systems within UK Ambulance Services. Experience of participating in and leading on clinical audits in the practice setting. Experience of coaching and mentoring. Proven background of building positive rapport with a diverse range of clinical colleagues, services and patients. Previous experience of working with Patient Group Directives (PGDs) or similar. Previous experience of remote working. Previous experience of supporting or managing change. Qualifications

Paramedic with current professional registration Educated to Diploma or DipHE level or equivalent demonstrable experience in practice Working towards or completed module in clinical assessment and diagnostics (or equivalent course) or equivalent experience in clinical practice Coaching & mentoring and/or leadership qualification Minor injuries / illness course or equivalent experience Prescriber Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £44,000 to £48,000 a year Depending on experience
