Arrival Medical Practice are looking for amotivated and enthusiastic Nurse Practitioner to join our friendly,patient focused practice team. Previous experience in minor illness and triage ina primary care health experience is essential. You must be able to demonstratesafe, clinical decision making for patients acting within professionalboundaries. Applicants must have completed their non-medical prescribing and be confident workingautonomously in primary care, as well as under the guidance of our GPs. The post holder will be an experienced NP who,acting within their professional boundaries, will provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care. They will demonstrate safe, clinicaldecision-making and expert care, including assessment and diagnostic skills,for patients within the general practice. They will demonstrate critical thinking in theclinical decision-making process. They will work collaboratively with thegeneral practice team to meet the needs of patients, supporting the delivery ofpolicy and procedures, and provide leadership as required. *There may be some cross site working required with Riverside Medical Practice* Main duties of the job

You will be joining a team of Two GP Partners, WTE2.5 Advanced Nurse Practitioners providing acute appointments, Practice Pharmacist, three Practice Nurses, a Health Care Assistant and two Trainee NurseAssociates. Clinically examine and assess patient needs from aphysiological and psychological perspective, and plan clinical care accordinglyby telephone triage or face to face consultations. Assess, diagnose, plan, implement and evaluateinterventions/treatments for patients with complex needs. Proactively identify,diagnose and manage treatment plans for patients at risk of developing along-term condition (as appropriate) Diagnose and manage both acute and chronicconditions, integrating both drug- and non-drug-based treatment methods into amanagement plan. Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice. Work with patients in order to support compliancewith and adherence to prescribed treatments. Provide information and advice onprescribed or over-the-counter medication on medication regimens, side-effectsand interactions. Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care. Support patients to adopt health promotionstrategies that promote healthy lifestyles and apply principles of self-care. Utilise and demonstrate sensitive communicationstyles, to ensure patients are fully informed and consent to treatment. Recognise and work within own competence andprofessional code of conduct as regulated by the NMC. Produce accurate,contemporaneous and complete records of patient consultation, consistent with Adhere to legislation, policies and procedures andparticipate in audit. Prioritise, organise and manage own workload in amanner that maintains and promotes quality. Assess effectiveness of care delivery through selfand peer review, benchmarking and formal evaluation Initiate and participate in the maintenance ofquality governance systems and processes across the practice and itsactivities. Use a structured framework (eg root-cause analysis)to manage, review and identify learning from patient complaints, clinicalincidents and near-miss events. Person Specification

Qualifications

Registered General Nurse: Registered with the NMC Experience of working autonomously Evidence of continuing professional development Experience

Ability to treat minor illnesses and ailments. Knowledge, experience, and ability to determine clinical diagnosis and facilitate appropriate treatment. Knowledge of and experience of managing long-term conditions. A passion towards the provision of high-quality care. The ability to work autonomously.
