This is a unique and exciting opportunity for an Occupational Therapist wanting to join the Complex Care and Review team due to expansion. The position is part-time (18.5 hours per week) and permanent, operating across the full Cheshire West and Chester footprint with a hybrid flexible working model.

About the Team The Complex Care and Review team is a growing, passionate and creative team consisting of a Team Manager, Practice Manager, 3 x Social Workers, 5 x Social Care Assessors, 1 x Occupational Therapist, and 1 x OTA. We work closely with the Adults with Learning Disability social work teams, Transitions team, CHC team, Mental Health teams, and colleagues in Commissioning.

Your Responsibilities You will have the opportunity to:

Undertake holistic OT practice including manual handling assessments.

Lead on major adaptations to enable people to live great lives.

Minimum Requirements You will have:

Degree or diploma in OT (Essential).

Up-to-date registration with HCPC (Essential).

Enhanced DBS plus Adults and Children's Barred List (Essential).

Experience of working in a Social Care and/or Health community setting.

Awareness of multi-disciplinary working and an integrated approach.

Experience of liaising with clients, their families/carers, and other professionals.

Demonstrable working knowledge of Health and Social Care Policy, housing, and legislation affecting OT practice.

Ability to assess and manage complex clinical risk.

Ability to cope positively with change and promote positive changes.

Self-motivated individual with a passion to make a difference in adult social care.
