This role will involve Nurse management, leading the nursing team, responding to changes in nursing practice and reporting to the practice manager and senior GP partner. The Practice is offering pastoral support to the post holder by way of mentorship with a senior established ANP. Clinical roles include, but not limited to Assess, plan, develop, implement and evaluate programmes which promote health and well being, and prevent adverse effects on health and well being Assess, plan, develop, implement and evaluate individual treatment plans for patient with a known long term condition Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition as appropriate. Undertakes agreed procedures eg - blood pressure, pulse rate and rhythm, temperature, height and weight body mass index, venepuncture, ear syringing, ECG, peak flow readings, cytology, travel and childhood vaccinations, interpretation of blood results via the pathology link Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiating effective emergency care. Review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence based practice and national and practice protocols Lead on wound care management, participate in immunisation programmes for both adults and children Lead on Nursing staff appraisals, day to day management of the nursing team.
