This role involves working in various clinics across our sites in the London Borough of Hillingdon to provide practice nursing appointments, or appointments in specific clinics for which you have the appropriate skills or training, with the potential for some home visits. There is the capacity to broaden the role to have additional input into audits, infection control, planning for clinics, training and support. About us

Key Responsibilities Working alongside GPs, healthcare professionals, and administrative staff in our community IV antibiotic clinic to assist with assessing patients, nursing care needs, and escalating concerns when needed. Carrying out basic clinical assessment and observations for patients requiring IV antibiotics. Preparing intravenous antibiotics and setting up IV infusions. Assisting with cannulation and phlebotomy. Aseptic techniques, cannula care, and dressings in keeping with infection prevention and control guidance. Ensuring there is an appropriate supply of medication and equipment to deliver a safe and effective service. In accordance with the service's needs, the post-holder will make him/her-self available to undertake a variety of duties including cervical smear tests, post-operative and basic dressings, HRT reviews, Pill Checks, Immunisations (Baby and Travel if trained), BP checks, Asthma and COPD checks, setting up intravenous drugs, diabetic reviews, Spirometry, and Warfarin Monitoring if trained. Assessing the health needs of patients with chronic health needs and using your own judgment to assess whether further input is needed. Screening patients for disease risk factors and early signs of illness. Contributing to coding requirements and recording data appropriately. Contributing to PCN requirements and recording data appropriately. In consultation with patients and in line with current practice disease management protocols, developing care plans for health-related illnesses. Screening patients for health education. Admitting and discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a practice nurse working within primary care, as set by Royal College Nursing and the Nursing and Midwifery Council and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups, and all other regulatory and standard-setting organizations. Additional Responsibilities The responsibilities listed below are not essential for the role but are desirable, although dependent on the qualifications you may or may not have. Collecting data for audit purposes and contributing to the practice audit programme as required. Taking responsibility for some aspects of the clinical work within the team, for example: coordinating aspects of governance, QOF, enhanced services, CPD, and keeping clinical protocols up to date. Working in clinics to provide enhanced services for patients in the community. Attending multi-disciplinary meetings to interpret results and plan for patient care. Personal & Professional Development: The post-holder will participate in any training program implemented by the Confederation as part of this employment, such training to include: Mandatory training such as basic life support and fire training Taking responsibility for own development, learning, and performance and demonstrating skills and activities to others who are undertaking similar work Communication: The post-holder will recognise the importance of effective communication within the team and will strive to: Communicate effectively with other team members. Communicate effectively with patients and carers. Recognise people's needs for alternative methods of communication and respond accordingly, including the use of interpreters. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team's performance. Work effectively with individuals in other agencies to meet patients' needs. Effectively manage own time, workload, and resources. Experience

Able to demonstrate ongoing work towards your professional development and revalidation requirements. Well-developed IT skills and an enthusiasm for new applications and software. Good time management and highly organized. Ability to use initiative, judgment, resourcefulness, common sense, and local knowledge. Able to work under pressure and to meet deadlines. Interest in quality improvement work. Recognizes priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors when appropriate. Able to work effectively independently and as a team member. Demonstrates accountability for delivering professional expertise and direct service provision. Qualifications

Secondary Care / Hospital Nursing Experience. Eligible to work in the UK. Full registration with the Nursing and Midwifery Council. Certificates for Practice Nurse Training and Cervical Smears Training with relevant registration numbers. Experience working within the NHS or Primary Care. Experience or training to work in Enhanced Services Clinics with any of the following specialisms: Travel Vaccinations and Advice. Spirometry and Reversibility. FENO. Cannulation and administering IV medications. Disclosure and Barring Service Check
