Marlow Adult Community Health Care Team (ACHT) is delighted to announce a Band 6 District Nurse/Community Nurse/Case Manager position: 1 x full time 37.5 hours per week The ACHT reflects an integrated approach that has been designed in response to the changing needs of the local population, with a special emphasis on proactive care for patients in the community setting, including caseload management, prevention of admission, reablement, and end-of-life care. Main Duties of the Job

Provide interventions tailored to meet the needs of the individual Contribute to processes for proactively case finding patients who are high intensity users of community and primary healthcare Assess the physical and psycho-social needs of the defined caseload Improve health outcomes using specialist nursing skills and knowledge Manage a caseload of patients with long-term conditions who would benefit from case management Person Specification

Education, Qualifications & Training

Registered Allied Healthcare Professional / Registered Level 1 Nurse Degree/Diploma in Health/Nursing Studies or related subject Evidence of recent Continuous Professional Development Skills, Abilities & Knowledge

Demonstrate specialist clinical assessment and evaluation skills Excellent written and verbal communication skills Ability to manage complex cases Knowledge of legal requirements for working with vulnerable adults Experience

Demonstrable experience of working in a community/in-patient setting for a minimum of 3 years post registration Experience of working within multi-agency/multi-disciplinary teams Special Circumstances

Valid and current driving license or able to travel to locations throughout Buckinghamshire PPE requirements:
