Croydon Health Services offers a truly integrated approach to paediatric nursing across our acute and community services, supporting seamless pathways for children and young people between the hospital and their home.

You will be managing children's acute and long-term healthcare needs between the primary, secondary, and tertiary interface. (Case management will be supervised by the Band 7 Community Children's Nurse as required).

You will work with and support families and other agencies, facilitating the delivery of nursing care.

Main duties of the job

Facilitate effective communication links between primary, secondary, and tertiary healthcare providers, and the voluntary and statutory sectors.

Be a children's nursing resource for families and professionals, providing one-to-one and group teaching sessions as required.

Have day-to-day clinical responsibility for the Children's Hospital at Home Team caseload to meet the individual needs of children and their families, in the absence of senior colleagues.

Be responsible for assessing, planning, implementing, and evaluating care, liaising and referring as necessary.

Be responsible for all liaison, action, and documentation required following nursing care interventions.

Person Specification Education and Training

Evidence of study at diploma or degree level.

Specialist practitioner Community Children's Nursing qualification.

Car Driver/current UK License.

Knowledge

Knowledge and awareness of acute and community children's nursing.

Knowledge and understanding of clinical governance.

Able to demonstrate understanding of current issues, policies, and frameworks affecting primary care and children's nursing.

Experience

Experience of working in Children's community nursing.

Teaching and supervising of parents and colleagues.

Experience of child protection procedures.

Special interest pertinent to Children's Community Nursing.

Skills

Ability to transfer nursing skills from hospital to home setting.

Ability to work as part of the primary and multidisciplinary healthcare teams.

Skills in evaluating caseload activity against the operational/business plans.
