Horton Park Medical Practice is looking to appoint a motivated, enthusiastic, experienced Practitioner to join our friendly clinical team. The Advanced Nurse Practitioner/Advanced Care Practitioner will work autonomously within their profession scope of practice providing acute care for patients via face-to-face, video, and telephone consultations and when required, home visits. They will need to provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care, making necessary referrals for investigations in the appropriate manner. The candidate will need to demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. They will need to be able to work collaboratively within the practice team to meet the needs of patients and support the delivery of policies and procedures. About us

The post holder is an experienced nurse who, acting within professional boundaries, will provide care to patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. To work as part of a clinical team which includes GPs in managing acute daily demand. To work collaboratively with the practice team to meet the needs of the patients, supporting the delivery of policy and procedures, and to develop a leadership role within an area of clinical practice. Experience

Experience of working in a community setting. Experience of managing minor ailments. Experience of working in a GP practice. General Nursing Experience across various chronic diseases. Qualifications

Recognised Advanced Nurse Qualification (MSc). Primary Care Experience. Nurse-led management of minor illness, minor ailments, and injuries. Nurse-led triage. Diabetes Diploma. Respiratory Diploma. Cytology. Disclosure and Barring Service Check
