The Integrated Discharge Team at Somerset NHS Foundation Trust focuses on delivering efficient, sustainable, and patient-centred discharge processes. Our goal is to ensure patients are discharged from acute hospitals with the necessary support to promote independent living or alternative care when needed. The team integrates multiple services, including the Transfer of Care Hub and Discharge Lounge, working across acute sites in Somerset to improve patient flow and outcomes.

We value our team members highly, so development and training within this role will be offered and prioritised.

There is an expectation that hours are worked flexibly over a rota basis, which includes day shifts and weekends.

Main duties of the job

As an Integrated Discharge Nurse, you will play a vital role in the daily operations of the Discharge Team, supporting the Senior Team in managing clinical and non-clinical aspects of the service. You will coordinate patient discharges, provide expert guidance, and ensure effective communication with internal and external stakeholders. Your primary focus will be to improve patient flow, reduce length of stay, and enhance patient experience by supporting seamless discharge processes.

Registered Professional Qualification Degree level education Evidence of post qualification training relevant to specialist area

Qualified RGN with previous experience of working at Band 5 or above in a health or social care setting. Experience of developing and implementing new services within the NHS. Evidence of autonomous working. Evidence of problem solving. Knowledge of acute and community care, social services, and primary care interface. Knowledge of relevant legislation relating to work area. Knowledge of the challenges surrounding complex discharge planning. Knowledge of rehabilitation and continuing care provision.

Desirable

Experience of multi-disciplinary working. Experience of staff management and undertaking skill mix reviews.

Additional Criteria

Essential

Able to understand the perspectives of numerous key stakeholders and align aims and purpose. Sound leadership skills.
