Main duties of the job
As an Integrated Discharge Nurse, you will play a vital role in the daily operations of the Discharge Team, supporting the Senior Team in managing clinical and non-clinical aspects of the service. You will coordinate patient discharges, provide expert guidance, and ensure effective communication with internal and external stakeholders. Your primary focus will be to improve patient flow, reduce length of stay, and enhance patient experience by supporting seamless discharge processes.

Qualified RGN with previous experience of working at Band 5 or above in a health or social care setting. Experience of developing and implementing new services within the NHS. Knowledge of relevant legislation relating to work area. Knowledge of the challenges surrounding complex discharge planning. Knowledge of rehabilitation and continuing care provision.
