Job Purpose:

The postholder will maintain the highest standards of nursing care with patients, their families, and carers in the In-Patient Unit. This role will involve providing holistic assessment, identification of patient needs, care planning, and care provision jointly with other multidisciplinary team members, so that people who use our service receive coordinated, compassionate, and person-centred care. The role requires a commitment to enabling choice and providing respectful and dignified support for people as they approach the end of life and to their families and carers. The postholder will contribute to a safe, effective, and efficient culture by participating in risk assessment and by working in accordance with Phyllis Tuckwell (PT) policies and procedures.

Responsibilities Clinical

To provide best practice using evidence-based care to underpin the delivery of safe, effective, responsive, compassionate care. To provide clinical guidance and support to staff, both paid and voluntary. To ensure statutory obligations are met and staff follow those that are relevant to the clinical area. To ensure quality standards are maintained and enhanced, and constantly re-evaluated in order to provide the highest standards of holistic care. To co-ordinate 24-hour provision of assessment, planning, implementation, and evaluation of nursing care. To work as part of the multi-disciplinary team, where there is a healthy regard and value for multi-professional knowledge and skills to enhance patient and family experience. To ensure patient records are properly maintained in accordance with NMC guidelines and PT policies. To demonstrate an understanding of issues relating to the ethical aspects of palliative care and their application in the workplace. To report accidents, incidents, and complaints to the Ward Sister/line Manager/or on-call Manager, and/or Medical Director where appropriate. To understand and adhere to the PT Medication Management policy in relation to the correct handling, usage, and administration of all drugs including controlled drugs.

Professional

To understand and work within the Health and Social Care Act (2012), the PT business plan, and philosophy. To work as part of the multidisciplinary team, providing holistic care to patients, their families, and carers centred on understanding their needs and preferences for care provision. To provide appropriate information and support to patients, carers, and friends in accordance with Caldicott principles. To liaise and develop effective relationships with external health care professionals and other agencies ensuring patient continuity across all care settings.

Managerial

To ensure a safe working environment is maintained, in accordance with the PT Governance, Care, Fire, and Health and Safety policies, reporting any potential hazards to the line manager. To report any feedback concerns and complaints from patients, their relatives, or other advocates to the Ward Sister, line Manager, or manager on call as appropriate. To ensure that the team has the appropriate skill mix and nursing resources are used wisely. To ensure team cohesion and spirit is maintained through ongoing supportive relationships, and consideration of colleagues at all times. To act up for the Nurse Key Worker as required, ensuring ongoing continuity and communication of care. To help support a culture of ongoing professional and personal development relating to clinical practice. In the community to adhere to the lone working policy, to underpin staff and patient safety.

Training and Development

Team

To develop an area of clinical expertise and disseminate to peer team members as a specialist resource. To support the professional development and safe practice of others.

Personal & Professional

To maintain up-to-date clinical experience and knowledge and be able to provide evidence of this, including for re-validation purposes. To act as an assessor and mentor to administrative personnel and students who need to develop skills and acquire experience in the field of palliative care. Through the use of the PT annual appraisal, encourage professional growth and identify areas for development. To strengthen and develop clinical skills across all care settings in conjunction with line manager.

Research

To participate in research and audit within the PT environment, encouraging staff participation. To implement findings of evidence-based practice.

Role holders are required to understand their responsibilities in the safeguarding context, ensuring that Phyllis Tuckwell reduces the risk of harm or abuse of adults and children at risk. You are required to adhere to Phyllis Tuckwell's Safeguarding Policy (held on the NEST), which supports the local authority's multi-agency safeguarding policy, the law (Care Act 2014), and Mental Capacity Act 2005 (including Deprivation of Liberty Safeguards). Inform the appropriate PT professional where there is concern that an adult or child may be at risk of harm, abuse, or neglect.
