Perinatal Community Mental Health Practitioner Hampshire and Isle of Wight Healthcare NHS Foundation Trust The Hampshire, Frimley and Isle of Wight Perinatal Mental Health Service is a well-established and pioneering specialist service consisting of an expert multidisciplinary community team and a mother and baby inpatient unit. The service is also an early implementer site for Maternity Mental Health Services, broadening the remit and profile of specialist interventions available for women/birthing people and ensuring an enhanced care pathway. The Perinatal Community Team provides a specialist community service for the assessment and treatment of women with severe mental health problems in pregnancy and up to 2 years postnatal.

We are looking for an experienced Mental Health Nurse, Occupational Therapist or Social Worker with an interest in the field of Perinatal Mental Health.

The available role is within our South West Hampshire locality team based at Fareham Community Hospital. A car driver is essential. Full-time or part-time working is possible.

Main Duties of the Job The role of the Perinatal Community Mental Health Nurse/Practitioner will be to undertake comprehensive assessments, manage a complex caseload of service users, provide practical advice and support including risk management, psychological interventions, and overseeing any safeguarding issues. Care coordination and care planning within the Multidisciplinary Team is a crucial part of this job to ensure patient-led, collaborative, high-quality care is being delivered.

Person Specification Qualifications

Registered Mental Health Nurse (RMN) qualification / Diploma or degree in Occupational Therapy / Certificate of qualification in Social Work / Diploma or Degree in Social Work (e.g., Dip SW, BA Social Work).

Driver's license: Use of suitable transport to carry out the job of community nursing within a rural and urban geographical area. Full driving license if driving own vehicle.
