Medicines Optimisation Pharmacy Technician- Discharge to Assess Pathways / Community Services Midlands Partnership University NHS Foundation Trust is unique in combining physical and mental health, learning disability, community and adult social care services in a single Trust across Staffordshire, Stoke-on-Trent and Shropshire supporting the provision of a truly integrated care framework across health and social care services. This is an unique and exciting opportunity to lead on the development and delivery of an Integrated Home First Medicines Optimisation Service to improve safe transfer and use of medicines across clinical interfaces; to meet all legal requirements and the Good Practice Standards prescribed by the Care Quality Commission (CQC) and other relevant agencies; to support safe administration of medicines in community settings; to meet the recommendations published by NICE NG 67 "Managing medicines for adults receiving social care in the community." The working hours are 37.5 hours per week Monday to Friday with limited late shift duties and Bank Holiday commitments. This provides a fantastic work life balance whilst being a valued member of the Pharmacy Medicines Optimisation Team. The post holder may be required to work flexibly to meet the needs of the service. Main duties of the job

You will work as part of a multi-disciplinary team within the North and South Home First Hubs and support the Lead Pharmacist in the establishment and management of care pathways for the referral of patients referred into the Home First Service with medicine-related problems for assessment, support and follow-up by a specialist pharmacy team, working across health and social care interfaces. This role will involve cross sector working in the community with multiple agencies and Primary Care Networks (PCNs) to support with complex medication reviews for patients in their own homes. This role will be extremely rewarding, and requires a motivated, dynamic individual who is experienced in working with a range of stakeholders and has a track record for service development and delivering to agreed targets. You will be a GPHC registered pharmacy technician with excellent interpersonal and IT skills. This is a permanent full-time post although part time hours or a job share will be considered. About us

Qualifications

BTEC and NVQ3 (or equivalent) in Pharmaceutical Science Registered as a Pharmacy Technician with the General Pharmaceutical Council Nationally recognised qualification as accredited Medicines Management Technician Nationally recognised Accredited Checking Technician Certificate Experience

Knowledge of the principles of medicines optimisation Experience in a broad range of hospital/community pharmacy activities, including dispensary management and clinical pharmacy activities Staff Management Disclosure and Barring Service Check
