Within this role, you will work with the team to provide a responsive assessment and home treatment service to those experiencing mental health crisis in the community over a 24-hour period. This is an exciting time to join the team as through investments and changes to the community mental health framework clinicians will have dedicated time working within rapid response and home treatment team. Job responsibilities

KEY RESULT AREAS: Work collaboratively with MDT members to contribute to a flexible and effective interdisciplinary team approach to clinical care. Communicate face to face with clients on a daily basis, some of which display challenging behaviour. Establish a therapeutic relationship with service users and their relatives/carers. Assess capacity and obtain informed consent from service users and work within a legal framework with those clients who lack capacity to consent to treatment. Undertake, record, review and work within individual risk assessment and management plans in line with service and Trust policy. Provide mental health assessment of clients with acute mental health needs, clearly assessing and managing risks. Lead joint reviews for patients with complex needs to plan transfer of care. Review patients for transfer to onward mental health support as per MDT plans. Ensure service users are involved in the planning and prioritisation of their care plans wherever possible. Adapt practice to meet individual service users' circumstances, including regard for cultural and linguistic differences. Work collaboratively with service users, staff, and relatives to identify goals and determine and deliver an appropriate intervention plan which aims to maximise engagement and performance whilst balancing risk, and promoting choice and independence. Monitor, evaluate and modify interventions in order to meet needs and to measure progress and increase efficacy for service user outcomes. Demonstrate and apply understanding of impairments to mental well-being and their impact on function, including strategies to manage impairments and potential risks. Identify clients' needs for additional care provision / housing/specialist placement/further assessment/referral on to other agencies. Act as a source of clinical reference for other colleagues regarding the service users, providing advice to enable and improve service/care provision. Attend and actively participate in relevant clinical meetings/forums. RESPONSIBILITY: Work across two functions - Rapid Response and Home Treatment Team providing assessments and interventions until the crisis is resolved. Screen and work with all newly referred service users experiencing acute mental health crisis providing assessment or short-term interventions until crisis is resolved. Provide short-term specific psychosocial interventions for those who would benefit from crisis interventions. Ensure documentation is reviewed and updated in line with quality improvement standards. Shift co-ordinate, lead the shift, allocate work to the wider MDT and prioritise workloads. Contribute to the planning and implementation of induction, training and education of students and staff. Contribute to the delivery of specialist/professional training packages of in-service training and staff development when necessary. Fulfil the role of mentor/practice educator, maintaining own update training and ensuring responsibility for regular placement provision and ensuring supervision and written assessment of students whilst on placement. Assist the band 7 senior practitioner by identifying personal and professional training needs in order to appropriately plan for future training and development. Provide effective supervision, support, and guidance for support/STR/peer workers and any work delegated to them. Responsible for the delivery of individual care plans for a caseload. Implement policies as directed by the trust/directorate for the department, ensuring awareness throughout the department. Participate in leading service developments. Communicate complex, sensitive and emotional information using a variety of mediums; being able to adapt methods/style/approach as appropriate. Person Specification

Qualifications

Degree or equivalent in Occupational Therapy Experience

Experience working in mental health service Disclosure and Barring Service Check
