Job Summary: A well-established and patient-focused medical practice in Chelmsford is seeking an experienced and dedicated Practice Nurse to join their dynamic healthcare team. The successful candidate will play a vital role in delivering high-quality care to a diverse patient population.
Key Responsibilities: Provide comprehensive nursing care, including wound care, immunisations, long term condition management and health screenings. Conduct patient assessments, develop care plans, and support the delivery of patient-centred care. Collaborate closely with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure continuity of care. Offer patient education and health promotion advice, empowering individuals to take control of their health and well-being. Maintain accurate and up-to-date patient records, ensuring compliance with all relevant guidelines and protocols.
