Approximately $71,703 - $111,642 per year. All individual pay rates are calculated based on the candidate's experience and internal equity. Master's Level with License: approximately $71,703 - $82,365 per year PhD Level with License: approximately $89,789 - $103,140 per year Overview of Role:

Develop a plan of treatment and provide direct therapy services to individual patients or families. ESSENTIAL DUTIES AND RESPONSIBILITIES: Provides clinical intervention and consultation to patients through scheduled therapy appointments or during a patient’s medical appointment at the clinic. Responsible for the assessment, treatment planning, and goal-oriented therapy services for established patients, including crisis intervention as needed. Maintains working knowledge of evidence-based, culturally appropriate practices for the treatment of trauma, substance use, and mental health, and familiarity with current DSM and ICD Diagnosis Codes. Demonstrates knowledge of universal screening and follow-up for depression and substance use. Maintains contact with clients in distress, including assisting with securing hospitalization. Acts as a consultant to care team members to promote whole person care for our patients, including crisis intervention, mandatory reporting, management of chronic illness. Completes documentation that satisfies minimum documentation requirements for all services rendered within a timely manner. Outreach patients to maintain engagement with care team, assist them to re-engage in care, and/or build motivation to initiate care. Coordinates care for complex patients with co-morbid mental and physical health conditions with the care team and other agencies providing services to our patients. Knowledge of community resources and ability to make appropriate referrals. Actively participates and utilizes feedback to improve patient care. Assists with implementation of an effective Quality Improvement Program. Other duties as assigned, including possibility of travel to other clinics for meetings or to provide direct services to patients. POSITION QUALIFICATIONS: Education and Experience: Licensed PhD or PsyD, Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage/Family Therapist (LMFT) required. Two years or more experience providing direct clinical services preferred. Knowledge, Skills and Abilities: Bilingual in English and Spanish required. Responsible for resolving conflicts in a professional manner and is a model for strong interpersonal communication skills with all staff. Excellent organizational, time management and problem-solving skills. Sensitivity to under-resourced, diverse communities. Clinica Family Health and Wellness is an Equal Opportunity Employer.
