Essex Partnership University Trust (EPUT) is currently looking to recruit HCA/Rehabilitation Assistants (Health Care Assistants in our rehab team) to join our community rehabilitation service. Main Duties of the Job

The Collaborative Care Team is a multi-disciplinary therapy-led team providing rapid assessment and rehabilitation for patients living within their own homes. The team supports the facilitation of discharge and the prevention of admission for patients who require rehabilitation for a range of conditions including stroke, orthopaedics and unstable fracture management; neurological issues, and jointly works with the Frailty Virtual Ward team and service. As part of our friendly supportive team, you will be providing person-centred care and supportive recovery. The service is provided across the South East Essex area including Castlepoint, Rochford and Southend, and as such, it is essential that any applicant is able to travel efficiently and effectively across this area. The successful candidate will support people with personal care and assist in the provision of rehabilitation programmes during a range of shift patterns over seven days per week, in line with service needs. Candidates should be motivated, willing to learn, enthusiastic, and demonstrate good interpersonal skills when communicating with patients, families, carers, and colleagues. The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our population’s health and wellbeing and reducing health inequalities. To support this ‘one workforce model’, the successful candidate for this post will be required to attend our 5-day (Monday to Friday) face-to-face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process. Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience.
