As part of our Community Transformation programme, we have an opportunity for a Registered Mental Health Nurse to join our established Multidisciplinary team. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders. Main Duties of the Job

Undertake comprehensive assessments and formulate personalised care plans. Conduct risk assessments of individuals and their environments. Provide and oversee the management and care of service users. Produce accurate and timely documentation and maintain records. Act as a Care Co-ordinator/Lead Practitioner. Provide education about prescribed medication and mental illness. Person Specification

Professional

Professional registration Evidence of post-qualification training Transport

Car driver with valid UK licence Own car for work purposes Experience

Minimum of 12 months experience at Band 5 Evidence of community exposure Please note:
