Band 6 Community Physiotherapist- West Sussex

An exciting opportunity has arisen for an experienced and motivated Band 6 Community Physiotherapist to join the UCR Team in West Sussex. This role will focus on providing high-quality care to patients in the community, specifically within respiratory and elderly care. Location: West Sussex Start Date: ASAP Hours: Full-time (Part-time considered) Hourly Rate: £25-£28 depending on experience As a key member of the team, the successful candidate will be responsible for assessing and managing a diverse caseload, ensuring that patients receive tailored rehabilitation and physiotherapy interventions to promote recovery, independence, and well-being. This is a fantastic opportunity to work within a supportive multidisciplinary team that focuses on delivering patient-centred care in the community. Key Responsibilities: Deliver physiotherapy services to patients with respiratory and elderly care needs, including home visits. Conduct comprehensive assessments and develop individual treatment plans. Work closely with a multidisciplinary team, including GPs, nurses, and other healthcare professionals. Provide rehabilitation and education to help patients manage their conditions and improve their mobility. Maintain accurate patient records in line with local and national clinical standards. Promote and support self-management strategies for patients. Liaise with other services and agencies to ensure continuity of care. Essential Requirements: HCPC registered physiotherapist. Proven experience in respiratory and elderly care physiotherapy. Full, valid UK driving license and access to a car for home visits. Strong communication and interpersonal skills, with the ability to work independently and as part of a team. A flexible and compassionate approach to patient care. Ability to manage a caseload effectively and efficiently. Desirable: Experience working in a community setting. Knowledge of reablement and rehabilitation strategies for older adults. This position offers an excellent hourly rate between £25-£28, depending on experience, and the opportunity to make a significant impact on the care and well-being of patients in the community. The role is available as full-time, but part-time options will also be considered.
