St Luke's Hospice in Basildon has a fantastic opportunity for a community nurse team leader, on a part-time basis for 22.5 hours per week. You will manage a team of community nurses ensuring the provision of high standards of care to patients and their families or carers who have a palliative care need. Main duties of the job

Providing leadership and guidance to the team in Hospice Community Services. Supporting the teams to ensure the delivery of the right care at the right time to individuals with a palliative care need in the community. Working, actively supporting and liaising with other health care professionals and agencies. Assisting in audit and research projects, particularly within the service, taking a lead on specified projects as appropriate. Monitoring and evaluating team performance, providing guidance and support where necessary. Fostering a culture of continuous improvement and professional development within the team. About us

Our care considers people's medical, spiritual and emotional needs, including support for loved ones and carers. Anyone living in Basildon and Thurrock can call on us 24 hours a day, seven days a week. Working alongside other healthcare providers, our specialist services are available at the hospice, out in the community or in a person's place of residence. Together, we fulfil people's choices at the end of their life. We are a charity and the services we provide are free. We rely on voluntary income from local people to deliver the care they deserve. Current NMC registration with an active PIN. Disclosure and Barring Service Check
