We are recruiting for a skilled Night Nurse/Paramedic to join our Urgent Care Service in East Kent. The East Kent Night Nursing Service provides care for patients overnight with an urgent clinical need such as end of life symptom management, verification of expected death and urinary catheter issues. Referrals are received from NHS 111, SECAMB, Out of Hours GP and directly from patients and relatives via our Local Referral Unit. After a period of induction and consolidation, you would be expected to rotate as the shift co-ordinator. This role includes triaging the calls and offering telephone advice or allocation for a clinician to visit. We are looking to recruit an experienced band 5 practitioner to the service primarily based in Ashford. The successful candidate would need to be comfortable with lone working and driving long distances across the geographical area. A full valid driving license and access to a car is essential. Main duties of the job

Your role will involve: Assessment, planning, implementation, and evaluation of holistic care The development of integrated care plans in partnership with the patient Ordering equipment and sending onward referrals as necessary Supporting in the supervision and development of all junior staff including Health Care Assistants and Students Actively working as a member of the multi-professional team to provide high-quality care to patients Undertaking a range of competent clinical interventions and identifying any deterioration in patients' condition, taking appropriate action Managing a caseload of patients on a daily basis consisting of patients with complex needs associated within a specific specialty Supporting in duty role and completing triages Additional Working Pattern: 38 hours per week About us

Qualifications

Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC or Physiotherapist; Occupational Therapist; etc / HCPC). Must have valid UK NMC PIN. Experience

Experience of recent community/acute healthcare gained through education or work experience placement or previous employment. Knowledge

Awareness of professional issues and recent developments in the NHS. Knowledge of up-to-date clinical practice. Skills & Abilities

Sound communication and interpersonal skills (verbal and written) Ability to organise own work Ability to cope with change Ability to use own initiative Holds current UK driving licence/permit & access to vehicle Personal Attributes

Able to support the implementation of local and national agendas for health under the guidance of the team lead / ward manager. Ability to prioritise care delivery in a professional manner, considering Trust policies and procedures. Disclosure and Barring Service Check
