The successful candidate will work collaboratively to address the child and family's needs; whilst working closely with colleagues from Social Care and Special Education Needs to provide a holistic approach to the support provided. The successful candidate will need to complete assessments as part of the ICB delivery of the NHS Continuing Care framework for the children in a full and succinct manner. These assessments are then presented by the team member to a multi-agency panel where eligibility will be discussed and agreed or declined. The successful candidate will be part of the clinical team where they will work independently managing their own caseload, ensuring that they fulfil the ethos of looking after the whole child.
