We are looking for a friendly and vibrant GPN to join our well-established nursing team. You will be NMC registered and will share in the usual distribution of work across our nurse team. Hours are up to full-time. Main Duties of the Job:

Chronic Disease Management/Long Term Conditions within the clinic setting. MUST have obtained Asthma, Diabetes, and COPD diplomas. Health Promotion To collaborate with and enable clients and other members of the primary health care. Cervical cytology & teaching breast awareness LES/DES/QOF related clinical and administrative work Routine immunisations / Childhood immunisations Travel health advice and vaccinations Contraception Flu, Pneumococcal, Shingles, and Pertussis in Pregnancy vaccinations Any other delegated duties appropriate to the post About Us:

Cervical cytology & teaching breast awareness Removal of Sutures ECGs LES/DES/QOF related clinical and administrative work Routine immunisations / Childhood immunisations Travel health advice and vaccinations Contraception Flu, Pneumococcal, Shingles, and Pertussis in Pregnancy vaccinations Chaperoning and assisting patients where appropriate who are being examined by another clinician At least 2+ years experience in Diabetes / Asthma / COPD Following agreed clinical protocols with referral to GPs as appropriate Clean and/or sterilise equipment using appropriate methods Maintaining and cleaning equipment used by the clinical team Maintaining clinical rooms, stocking and rotating items as required Maintain general tidiness and cleanliness of nurse treatment rooms Ordering of vaccinations and equipment to maintain stock levels Participation in administrative systems in the Practice At all times there will be a need to maintain accurate records A duty to advise colleagues of potential problems or errors within the range of assigned tasks Attend and participate in any Practice meetings when required Any other delegated duties appropriate to the post Person Specification:

Qualifications:

Registered General Nurse (diploma or degree level) Cervical Screening, Childhood Immunisations, Health Promotion Qualifications and Experience in: - Heart Disease, Travel Advice Non-medical prescribing Other:

Car driver with a clean licence Able to work at the desired times. Flexible and able to cover for colleague absence through sickness, annual leave, or study leave. Experience of primary care Experience:

Experience of working in accordance with the NMC Code of Practice Experience and competent in General Treatment Room Duties Practice Nurse Qualification/Experience Experience in Chronic Disease Management (Diabetes, COPD, Asthma, CKD, Stroke, Hypertension) Current BLS (within the last 12 months) emergency resuscitation training certificate. Experience of EMIS Web Clinical System Experience of Docman 10 General:

Ability to work without direct supervision and determine own workload priorities. Ability to work as part of an integrated multi-skilled team. Able to work under pressure Ability to use own judgement, resourcefulness, and common sense. Willingness and enthusiasm to develop nursing skills.
