We are looking for a dynamic, passionate, and motivated nurse with experience in Community Nursing to be a Team Leader within the Falmouth/Penryn Community Nursing Team. The post holder will work with the team to ensure provision of a holistic nursing assessment and high-quality, safe, evidence-based care to patients and carers, aiming to maintain patients in their own homes. As an autonomous practitioner, the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. They will manage a clinical caseload within a community nursing team with guidance from the Community Nurse Team Leads, Community Matrons, and Integrated Care Team Leads. This includes physical examination, detailed history taking, diagnosis of deterioration in pre-existing conditions, and treatment planning. The post holder will focus on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings. They will implement personalized care plans and collaborate with the whole community resource to facilitate ongoing care management. To work in partnership with patients, carers, and members of the multi-disciplinary team to offer support and advice, promoting informed choice; maximizing independence and optimizing quality of life. This post holder will be required to provide clinical management and leadership to all staff within the team. About us
