Main duties of the job Join us for a unique one-year programme designed to build a solid foundation in general practice nursing and make a difference to deliver essential nursing interventions, playing a key role in the local healthcare network. You will be part of a skill-mixed team, gaining invaluable experience to integrate within our practice and Primary Care Network.

Assessing patients and planning, implementing, and reviewing treatment programmes.

Working unsupervised to Patient Group Directions, agreed protocols, evidence-based practice, and within defined competencies.

Chronic disease assessment and management (Diabetes, Asthma, COPD, CHD)

New patient health checks and NHS health checks

Routine, travel, and child immunisations

Women's and men's health/ lifestyle advice and appropriate referrals

Contraception advice and safe sex guidance

Support for patients in managing chronic long-term conditions

Job responsibilities KEY DUTIES

To supervise junior members of staff

To supervise the delegation of work within the team and review performance.

To establish excellent working relationships with all service users and actively encourage and promote joint working in the pursuit of continuous service improvement.

CLINICAL & PROFESSIONAL RESPONSIBILITIES

Recognise and work within own competence and professional code of conduct as regulated by the NMC.

Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition, as appropriate.

Undertake opportunistic screening when clinically required.

Perform occasional venepuncture and electrocardiography (following appropriate training).

Undertake wound care/ management/dressing techniques.

Carry out ear examination and syringing, (following appropriate training and assessment).

Undertake male, female and family planning assessments, give advice and refer patients appropriately (following appropriate training).

Perform cervical screening and promote breast awareness (following appropriate training).

Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs.

Provide an adult and childhood immunisation service working to Patient Group Directions (following appropriate training).

Deliver travel health care, including vaccinations and advice for patients (following appropriate training).

Work with patients to support compliance with and adherence to prescribed treatments.

Support patients to adopt health promotion strategies that promote healthy lifestyles.

Prioritise health problems and intervene appropriately to assist the patient in urgent or emergency situations.

Undertake first aid and management of emergencies.

Apply infection control measures when collecting and handling laboratory specimens.

Contribute to the maintenance of the practice diseases registers.

Recognise the signs of child abuse and family violence.

Ensure that current evidence-based approaches to patient care are standard practice.

Support the practice in achieving quality indicators and targets.

Undertake accredited training in specific chronic disease management as required.

Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques.

Communication

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate effectively with patients and carers.

Maintain effective communication both within the practice environment and with external stakeholders.

Ensure awareness of sources of support and guidance.

In conjunction with the Practice Management team ensure the maintenance of equipment and stock levels relevant to patient care are suitably maintained and ordered.

Prioritise, organise, and manage own workload in a manner that maintains and promotes high quality service delivery.

Deliver care according to NICE guidelines and evidence-based practice.

Assess the effectiveness of care delivery through self and peer review.

Initiate and participate in the maintenance of governance systems, audits and processes.

Assess and report on the impact of policy implementation on the delivery of care.

Use a structured framework to manage, review and identify learning from patient complaints.

ADMINISTRATIVE RESPONSIBILITIES

Lead on key aspects of quality assurance to develop robust outcome indicators.

Participate in partnership programmes of awareness raising.

Produce accurate, contemporaneous, and complete records of patient consultations.

TEACHING & TRAINING RESPONSIBILITIES

Teach, support and mentor other staff to enhance their clinical practice.

Provide mentorship and guidance for health care assistants.

Develop, deliver, and evaluate training and education packages.

Provide education to patients, carers, families, and colleagues.

Maintain a level of expertise in specific chronic disease management.

Collaborate proactively with public health agencies.

Assist the GP's and ANPs to help deliver effective implementation and management of patients.
