The Community Children's nursing team is part of an integrated network of services for children in Bexley and Greenwich, providing high quality specialist child-centred care. The team of specialist and children's community nurses provides highly skilled nursing care to children and families with nursing needs in their own homes or community settings. To provide a high standard of evidenced-based nursing care for children and young people with the following conditions: acute and long-term conditions, children with disabilities and complex conditions including those requiring palliative and end-of-life care. To provide clear visible leadership and professional clinical expertise to the Band 5 Children's Community Nurses in the whole team to ensure every aspect of the child and family's experience is delivered in a safe, efficient, and evidence-based manner. This post is suitable for an experienced children's nurse who should have an interest in or a willingness to develop and extend specialised skills and knowledge. The applicant should have leadership skills and experience in either community or an acute setting. Evening and weekend shifts will be required on a pro-rata basis. Main duties of the job

You will hold a caseload of children who have varied health conditions, ranging from children and young people who have complex medical needs, long-term health conditions, and short-term health conditions requiring a short-term referral under the remit of our H@H service. You will support more junior staff members to develop skills and confidence in the role of the CCN. You will work closely with the band 7 specialist nurses who will support your learning and development within the scope of your role as a band 6 nurse. About us

You will work within an integrated Children's Nursing team covering both CCN & H@H; the role as a community children's nurse is highly varied. We are one large team covering both boroughs of Greenwich & Bexley. The caseload is varied with referrals of children and young people who have complex medical needs, long-term health conditions, and short-term health conditions requiring rapid service input to enable children and young people to be discharged from hospital early.
