The post holder will be expected at all times to work within the NMC Standards of Proficiency for Nursing Associates to meet with individual needs of the designated patient population. The post holder will be accountable to the registered nurse in charge of the ward/department for the delivery of nursing care to a specific group of patients. The post holder will organise own time and that of junior staff, delegating to junior staff and learners, and participating in teaching and education programmes for staff and patients. We require a Band 4 Associate Practitioner to work independently under the guidance and direction of a Registered Nurse in order to provide efficient and effective evidence-based nursing care to patients in their own homes. Applicants must have a Foundation Degree in Healthcare Practice/Adult Care as well as excellent clinical, communication and team working skills. Experience working in a Community Team is desirable. We offer a comprehensive induction programme and excellent ongoing professional development opportunities.

All candidates must have a clean driving licence and use of a car for work purposes. Once recruited staff can take advantage of our childcare facilities.

Qualifications and training

Must be able to speak, read and write English Must have clear legible handwriting Foundation Degree in Healthcare Practice/Adult Care Willingness to undertake further training relevant to this post Recent experience working as a health care assistant within Community Nursing Service or Primary Care Knowledge and Understanding

Must have knowledge and understanding of Health & Safety responsibilities. Understanding of the need for confidentiality in relation to work activities. Understanding of Equality and Diversity in relation to work activities. Understanding of the Associate Practitioner role. Understanding of principles of promoting independence. Aware of own level of knowledge and competency and of when to seek guidance from senior colleagues. Understanding of Child and Adult protection needs and action to take in event of concerns. Experience of working as part of a team. Knowledge of the provision of care for people with long-term conditions and/or end of life care. Experience of acting as a role model and mentor for others. Skills and Abilities

Must be able to follow standard operating procedures and written care plans. Must be able to demonstrate clear verbal communication skills with patients, carers and staff at all levels including feeding back information. Ability to prioritise own workload and organise time effectively. Ability to work without direct supervision, e.g. when in patients' homes. Ability to use initiative. Ability to keep calm and logical in emergency and stressful situations. Ability to work in partnership with patients and their families, respecting their choices and preferences. IT skills (e.g. data entry, Word, email, use of internet). Ability to deal with challenge and conflict in an appropriate manner. Evidence of ability to carry out administrative tasks. Ability to maintain accurate, concise and contemporaneous records and other relevant written documentation. Ability to work effectively and cooperatively within the primary health care team and with others within a diverse population. Experience of carrying out venepuncture and other monitoring/diagnostic tests, e.g. BP and BGM and other nursing duties. Ability to supervise and teach others. Other

Car driver with a current UK licence and access to a car for work purposes where required. Ability to work flexibly including unsocial hours and weekends where required.
