Role Summary An exciting opportunity is available for a passionate, enthusiastic, dynamic and motivated Care Coordinator with experience and a special interest in delivering excellent patient care and communicating with a team. The Croft Practice is a busy GP practice, currently expanding our patient list in preparation to move into our new building in early 2025. Key Responsibilities and Accountabilities Patient welfare is at the heart of this role, you will be communicating daily with patients, care homes and other NHS services, acting as a central point of contact. Organising the weekly MDT, liaising with the GP Partners and third party groups. Telephone communication – You will be coordinating arrangements for patients with long term conditions including learning disabilities, dementia, elderly, frail etc., as well as patients in a care home. This includes care planning with a person-centred approach and respect forms. Communicating with patients to ensure understanding of their changing needs, and addressing them. Ensuring SystmOne and patient records are updated accurately and in a timely manner. Appointment booking – Appropriate use of our clinical system (SystmOne) in booking appointments, booking remote appointments, liaising with other organisations where required, processing patient queries and logging confidential information. Treating all patients with dignity and respect, maintaining confidentiality at all times. Complete administration for all private work, supporting the Clinical Team to prepare the paperwork, organise payments and book any appointments required. General administrative duties including (but not limited to) subject access requests, using iGPR software. Using CAS to organise the incoming and outgoing of patient medical notes as per the need of the Practice. General To work within The Croft Practice’s mission and values. To commit to own personal development and attend training or development activities as required. To work in accordance with all relevant legislation. To undergo regular supervision and annual appraisal. To undertake any other duties as required, and as appropriate to the post.
