Commensurate with Experience. The Palliative Care Program is dedicated to delivering comprehensive home nursing services tailored to meet the unique needs of individuals, families, and groups facing end-of-life circumstances. Our team of skilled nurses is committed to elevating the standard of care for palliative clients, with a primary focus on proficient symptom and pain management. Through our specialized services, we aim to enhance the overall quality of life for individuals receiving palliative care. Please note that this is a visiting nurse opportunity; regular travel and driving are integral components of the position. We are looking for those who live in the Durham region, Scarborough, or North York area. There is a $2000 Sign-On Bonus for full-time nurses in Durham! Key Areas of Accountability: Assess, monitor, and make care planning/care coordination. Evaluate care plan interventions through ongoing reassessment and evaluation results. Ensure the quality of care of patients by identifying and acting on factors which affect practice autonomy and delivery of quality care. Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families. Support client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement. Inform nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and submit appropriate documentation to the case coordinator. Maintain documentation standards according to the College of Nurses of Ontario. Required Qualifications: Registered and in good standing with the College of Nurses of Ontario (CNO). Membership of your Professional Association preferred (RNAO/WeRPN). RPN diploma/degree or RN degree from a recognized university or college. Valid First Aid or CPR Certification. Preferences are given to candidates who have completed their Fundamentals of Palliative Care, LEAP, and/or CAPCE courses or are willing to work on these certificates within 1 year of hire with VHA. Acute Care and Medical-Surgical nursing experience is an asset. Knowledge of community nursing is an asset. Demonstrate effective communication (written and verbal) and documentation skills. Demonstrate reliable attendance, punctuality, and availability for the role. Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
