To work primarily with the Older Persons and Medicine Team, which includes working across multiple wards and community settings as required. This role involves collaborating with therapists to plan and facilitate patients' progression through their treatment plans, in accordance with the rehabilitation protocols set out for each patient.
Responsibilities include: Implementing prescribed treatment programmes for an allocated caseload, following an initial screening by a registered therapist. Taking responsibility for a delegated caseload and reporting outcomes appropriately, including urgent and important information to the appropriate person. Prioritising and planning your own daily programme of work.
