We have an exciting opportunity for a Cardiac Rehabilitation Nurse to join the Stepping Hill Hospital Cardiac Rehabilitation Service. The post holder will work within the multi-disciplinary team flexibly, to assist in the delivery of a high-quality service, providing expert support and advice to patients with Coronary Heart Disease or post Cardiac Surgery. The post will involve in-patient ward visits, on-ward referral to other services, performing clinical assessments, case management of patients, and assisting in the Cardiac Rehabilitation exercise classes.

Key Responsibilities: Provide specialist nursing assessment, advice, rehabilitation, support, information and development of evidence-based care pathways for patients following an ACS event or cardiac surgery. Be professional, legally responsible, and accountable for all aspects of your own work. Ensure a high standard of clinical care for the patients under your management and support more junior staff to do the same. Undertake all aspects of clinical duties as an autonomous practitioner. Case manage patients and own clinical workload. Provide telephone support to patients upon discharge from acute care. Undertake a comprehensive assessment of cardiac patients by using investigative and analytical skills to formulate individual management and treatment plans. Provide counselling, goal setting on risk factor modification and interpretation of clinical information (e.g., Echocardiogram reports) to enable risk stratification for appropriate delivery of care. Supervise the Cardiac Rehabilitation exercise programme alongside the Exercise Specialist. Provide group education to patients attending the supervised classes. Communicate efficiently and effectively with the wider MDT, Consultants and GPs. Work in line with the BACPR (British Association for Cardiovascular Prevention and Rehabilitation) Standards and Core Components. Represent the trust externally (locally, regionally and nationally) regarding services provided to patients with cardiac conditions.

If successful, this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary. In addition to this, it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
