Main area: GERIATRICS Grade: Band 7 Contract: Permanent Hours: Full time - 37.5 sessions per week Job ref: 162-6736-TA Site: Frailty Virtual Ward Town: Romford Salary: £51,883 - £58,544 per annum inclusive Salary period: Yearly Closing: 15/01/2025 23:59 An exciting opportunity has arisen on the Frailty Virtual Ward for a Band 7 Clinical Nurse Specialist - Frailty. The Frailty Virtual Ward is a 30-bedded capacity community-based service that aims to support patients who would otherwise be in hospital to receive acute care, remote monitoring, and treatment they need in their own home or usual place of residence. You will support your team, department, and organisation to achieve both trusts' values in your day-to-day work. The role involves working closely with the Acute Frailty and Virtual Ward multidisciplinary team (MDT) and is pivotal in ensuring all patients within the Virtual Ward receive the best possible care and service. This new post is available for a dynamic and experienced Nurse with an interest in providing care to patients suffering from frailty. You will work autonomously across the BHR area, including within the Acute Frailty & emergency departments, as well as visiting patients in care homes and the community, providing a range of services using your enhanced clinical assessment and treatment skills to assess and manage patients. Main duties of the job

This will require staff with skills and understanding. The applicant needs to have a good knowledge base around the needs of these patients. The applicant will be expected to demonstrate excellent communication and motivational skills to continue the excellent teamwork currently displayed. Expectations will include creating a ward that is supportive to all staff, particularly those who are new and junior, and to continue to develop an environment of quality and innovation, encouraging independent thinking and creativity. Arrange services and support with other healthcare providers. Act as a referral agent by establishing multi-agency collaboration across primary/secondary interfaces to meet the needs of the patient, including social needs. Perform point-of-care testing and obtain supporting information to inform the assessment of an individual. Provide clinical interpretation from clinical investigations and determine a treatment plan for an individual. Person specification

Education/Qualifications

BSc Nursing or working towards Diploma in Geriatric Medicine Independent/Supplementary Prescribing (V300) Skills/Abilities

Clinical leadership and decision-making skills. Ability to negotiate effectively with different disciplines and all grades of staff. Full Driving licence. Experience/Knowledge

Experience of teaching medical/nursing staff and AHP. Good communication skills, knowledge of patients living with dementia and frailty. Demonstrate up-to-date knowledge of best practice and current research in acute elderly/frailty and medical care. Experience in a Virtual Ward environment. Experience of quality improvement process implementation. Experience of acute & community working. All new staff appointed at the Trust are subject to a probationary period.
