Job Purpose The post holder will manage an Urgent Care Caseload within a multi-professional clinician-led environment, dealing with a wide range of presenting undifferentiated conditions. The post holder will work alongside a multi-disciplinary team of clinicians with acute and primary care first contact experience, including Nurses, Paramedics, Health Care Support Workers, Emergency Care Practitioners, Radiographers, and Paediatric Nurses, to assess undifferentiated urgent care patient presentations.

The post holder will provide clinical assessment and deliver treatments to service users who have undifferentiated health care requirements to support the services delivered by the primary care team and prevent unnecessary acute hospital attendances. They will utilise advanced clinical skills and competencies in health assessment, medicines management, leadership, and case management to maximise patient health and reduce risks contributing to ill health, thereby reducing unnecessary admissions to acute services. The post holder will participate in the development of a specific clinical area, e.g., Paediatrics, Sexual health, Safeguarding, and be an autonomous practitioner who is flexible in their role.

Participate in training opportunities for self and others within the UTC. Support and deliver training to UTC staff, staff within CityCare, and the wider healthcare community. Provide mentorship and support to UTC staff and students (including medical, nursing, and allied health professionals). The post holder will take operational charge of a shift on a planned rota basis in the absence of the UTC Coordinator and provide line management to non-registered staff.

Dimensions To work with the CityCare multidisciplinary team in the provision of primary healthcare to residents and visitors to Nottinghamshire. To represent Nottingham City Care Partnership locally, regionally, and nationally if required.

Clinical Responsibilities Comprehensively assess, review, and evaluate the needs of service users with undifferentiated healthcare needs to improve their physical, psychological, and mental health wellbeing to promote self-care and reduce the need for hospitalisation. Accurately prioritise patients within the service, undertaking clinical assessment and providing necessary treatment, including the use of Patient Group Directions in line with local/national policies, procedures, and guidelines. Carry out diagnostic tests only where clinically indicated, in line with organisational, local, and national guidelines and appropriate to the Urgent Treatment Centre setting. Develop a plan of care in conjunction with the service user through the application of clinical knowledge, analysis of symptoms and data, identification of risk factors, recognition of early signs of acute illness, and involving patients and carers in the care plan. Document safety netting plans and processes, and utilise effective communication to aid onward referrals to specialists as appropriate. Facilitate a coordinated primary healthcare response to service users' needs.

Work in an autonomous manner without direct supervision while recognising the limitations of the practitioner role. Carry out enhanced clinical skills as required, which may include suturing and plastering. Keep accurate and immediate records of all interventions, fully utilising software systems in place. Encourage clients to carry out self-care through effective communication of healthcare using written information as appropriate. Advise clients on the most appropriate use of Primary, Secondary, and Emergency services, and maintain patient confidentiality as per Urgent Treatment Centre and CityCare clinical guidelines.

Professional Responsibilities Work within the relevant professional code of conduct. Act in accordance with and promote the CityCare values at all times. Act as an ambassador for the Urgent Treatment Centre. Be committed to working flexibly within a dynamic environment, responding positively to the changing needs of the service. Promote and facilitate evidence-based practice, providing a high level of customer service and patient experience. Maintain professional awareness and attend appropriate training as necessary. Participate in the appraisal process and development of a Personal Development Plan, including clinical supervision and revalidation where appropriate. Develop a culture within the Urgent Treatment Centre team of continuing professional development, supporting staff in meeting the requirements for post-registration education and training. Provide clinical support and mentorship to other clinicians, nurses, and primary care assistants to meet their personal/professional development plans, including mentoring nurses through work-based learning programmes. Support/facilitate the provision of Clinical Supervision in accordance with organisational policy. Liaise with other agencies as appropriate to ensure comprehensive and collaborative care for service users.

Continuous Personal Development Maintain current professional registration and ensure that all personal responsibilities in relation to this and the revalidation process are met, including maintenance of a professional portfolio, evidence of practice hours, professional development planning, and reflective writing. Meet the organisational requirements for mandatory training appropriate to the role. Recognise personal professional limitations, practising accordingly, and seek advice and support as appropriate. Take an active role in the organisational appraisal process, developing an appropriate and relevant personal development plan and work-based objectives. Be aware of relevant organisational and professional safeguarding policies and guidance and act in accordance with their direction, attending update training as required.

Management Responsibilities Participate and assist in evaluation, audit, and research as required. Participate in the investigation of any complaints, incidents, and accidents. Participate in the training and induction of new members of staff to the Urgent Treatment Centre. Participate in the mentorship, support, and training of individuals on recognised training programmes, such as Student Nurses, Practice Nurses, and other pre and post-registration students.
