This is an exciting opportunity to work as an integral member of the Pre Assessment Team, providing advice and support to patients who have diabetes and require surgery at James Paget University Hospital.

The perioperative diabetes pathway was implemented with Get It Right First Time to improve patient safety, reduce post-op complications and improve patient experience. Support is provided throughout the perioperative journey with the majority of patient contact occurring before surgery as outpatient telephone consultations and in-person clinic appointments such as diabetes medication/ Insulin starts, followed by Inpatient post-op ward reviews when required.

This post is suitable for applicants with knowledge and clinical experience in caring for people with Diabetes.

The successful candidate will have excellent clinical, organisational and communication skills. Interest in education and enabling service improvement will be advantageous.

Main duties of the job

- To provide leadership to the Diabetes Pre-Assessment team to provide clinical support and expertise to Diabetic patients within the outpatient departments. - To act as a Diabetes nursing resource giving advice, education and psychological support to patients, carers and healthcare professionals regarding diabetes and the treatment and management of diabetes and its complications. - To review nursing practice and support the implementation of appropriate changes based on research/best practice. - To lead in the development and implementation of audit, quality and Risk Management initiatives. Specific focus will support improving the quality of care for patients who are undergoing elective and emergency surgery.

- To be an active member of the diabetic team and the wider professional agencies which are involved in the provision of diabetes services within the HealthEast CCG area.

- To promote standards of care and practice in line with Trust Policies and Guidelines.

- To assist in the maintenance and development of systems and strategies to promote and enhance the delivery of care to the patient group.

- To actively engage in promoting the best standards of clinical care and cost-effective delivery of that care including medicines management and resource utilisation.

- Non-medical prescriber is a requirement of the role for the band 7; if the successful candidate does not have the qualification, a band 6 position will be offered.

Job responsibilities

- To manage a clinical caseload of diabetic patients requiring surgery. - To provide clinical advice, education and support for patients, carers and other healthcare professionals in the treatment and management of diabetes and its complications. - To ensure that agreed standards of nursing practice are maintained. - To perform ongoing monitoring of Diabetic patients, under the direction of the Consultant Endocrinologist or medical physician. - Actively support/enhance good communication between members of the multi-disciplinary team, the patient and their carers. - To maintain accurate patient records and adhere to Trust Information Governance standards and policies. - To actively participate in the collation of data for research and audit purposes.

Person Specification

Education and qualifications

Essential

Registered Nurse/ Diploma in Nursing or equivalent Evidence of ongoing professional development Non-medical Prescriber qualification (band 7 requisite)

Desirable Qualification relating to Diabetes Degree or working towards

Experience and knowledge

Essential Broad post-registration experience with an interest in the field of Diabetes

Desirable Current or recent experience within a Diabetes team NHS experience Experience of leading/directing individuals

Skills, abilities and competencies

Essential Broad knowledge base including Diabetes and Long Term disease Excellent communication and interpersonal skills Ability to prioritise own workload Good organisational and team building skills Accurate document presentation and attention to detail Basic IT skills

Desirable Ability to implement change Knowledge of NHS computer systems Flexible approach to work in accordance with service demands Experience in patient education
