We are seeking a dedicated and experienced Senior Liaison Worker to join our team in the Bridgwater locality, delivering a high-quality service to individuals with mental health challenges who do not meet the criteria for secondary care. This pivotal role involves managing a small caseload while offering structured guidance, supervision, and support to mental health staff and village agents working with this group of service users.

This is an exciting opportunity for a skilled professional with a passion for mental health care to make a significant impact on the community and support a collaborative, person-centered approach to service delivery.

Main duties of the job

Our Community Mental Health Team is a Monday-Friday, 9am-5pm service, so no weekend working required office hours only!

You will manage a small, rolling caseload of patients allocated to you where you will monitor their mental state, promote independence, promote social inclusion, aid the recovery process, and reduce hospital admissions.

You will be a non-medical prescriber and support medication advice to patients.

You will work in collaboration with the Operational Team Manager, Clinical Leads/Practitioners and have close working relationships with the community teams, inpatient services, and the home treatment team across the service.

You will need a full driving licence with permanent access to own transport with appropriate business insurance this is so that you may carry out patient visits - you are able to claim expenses.

Qualifications

Essential

RMN To be registered as an independent non-medical prescriber with their respective professional regulating body Commitment to Continuing Professional Development in the field of mild, moderate and severe mental and emotional health.

Experience

Essential

Significant experience in working with people with complex clinical and risk presentations, including those with mental and / or emotional health difficulties. Experience of offering clinical supervision to other professionals Working with clients in the community

Additional Criteria

Essential

Ability to build collaborative therapeutic relationships with service users who are difficult to engage. Skills for assessing service users and carers and interpreting service user and family situation, and ability to draw together appropriate case formulation. Ability to work within and between multi-disciplinary teams and the wider Somerset NHS Foundation Trust and voluntary sector providers with the ability to develop and maintain communication on complex matters, issues and ideas and/or in complex situations where the atmosphere may be highly emotive. Communicates highly complex condition related information to service users & carers. Ability to offer highly specialised clinical supervision to other staff who may be of a different professional discipline. Assess, develops and implements psychosocial interventions provides specialist advice to service users, carers, individual team members and teams. Ability to undertake and communicate complex risk assessments
