The Role
We're looking for a dedicated Care Coordinator to join our team at Deptford Surgery. You'll work closely with both clinical and non-clinical teams to coordinate the care of our most complex patients. By collaborating with patients, families, and local care systems, you'll help provide holistic, patient-centred care. You'll report directly to the Lead Care Coordinator on-site and work in a dynamic, supportive environment.

Location: Deptford Surgery (SE14)
Hours: Full-time (37.5 hours/week)
Salary: Up to £27,000 per year, based on experience
