We have an exciting opportunity for a Band 6 Physiotherapist to join our Integrated Community Stroke Service in our West Somerset Team, providing inpatient and ongoing community stroke rehabilitation.

The successful applicant will manage a clinical caseload of stroke patients, lead and mentor junior physiotherapists and work alongside a multidisciplinary team, including Occupational Therapists, Nurses, Speech and Language Therapists, Psychologists, and Rehabilitation Assistants.

Experience in stroke care is essential, as well as excellent communication skills. You will support the development of the stroke physiotherapy workforce, ensuring high-quality care across the county.

The successful applicant will be supported by the Band 7 Clinical Specialist Physiotherapist and have access to ongoing development opportunities.

Applications are welcome from Band 5 physiotherapists who are looking for a developmental role.

The post is for 22.5 hours per week including bank holidays and weekends, with exact working pattern to be established at the time of appointment.

Main duties of the job

To use specialised skills to provide assessment and treatment of stroke patients, taking account of other pathologies and associated complex needs. To work within the rehabilitation and community settings. To take an active role in the development of integrated stroke rehabilitation services within Somerset. To provide a high standard therapy service to all patients referred to the service. To develop and deliver an individualised treatment programme. To agree patient centred goals in partnership with patients, carers, and other members of the healthcare team. To provide specialist advice, teaching and training regarding rehabilitation management of patients on caseload. To work as an autonomous practitioner having mainly self-directed support and learning, and formal agreed supervision according to Trust policy.

Person Specification

Qualifications

Essential

Diploma/degree in Physiotherapy. HPC registration. CPD - evidence of ongoing, dynamic and position relevant continuing professional development.

Desirable

Post graduate accredited course in Stroke rehabilitation Care of the Elderly and/or Rehabilitation/Neurology. Membership of the Chartered Society of Physiotherapy (CSP).

Experience

Essential

To have gained effective experience of stroke rehabilitation from student placements or post-graduate experience.

Desirable

Experience of lone working. Experience of working as part of a multidisciplinary team. Experience of service development and review.
