The Hospital at Home South Team is an innovative and pioneering service based at the Abingdon Community Hospital in Oxfordshire and covering a large catchment area across Central, West, and South Oxfordshire. In collaboration with colleagues based at the Oxford University Hospitals, we provide care and treatment to patients within their own home for those seeking to avoid an acute hospital admission or expedite a discharge from the Acute sector.
The service provides a holistic approach to assessment and treatment of patients within their own homes, for a variety of conditions commonly only previously treated with an acute admission.
You'll need to be patient-focused, flexible, able to work independently within a busy multi-professional team, as well as being proactive with a passion for community-based care. This is a pivotal role working with, in, as well as leading a strong and supportive team.
The service runs 08.00 – 22.00, 7 days a week, 52 weeks of the year, and requires some flexibility across working patterns.
Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
Responsibilities:
Assess and clinically review patients for a variety of acute conditions within their own home. Provide and follow a patient-centred treatment plan that has been agreed with the patient and the Consultant. Escalate any concerns in a timely and appropriate way. Provide team direction and caseload management as the daily coordinator. Provide high-quality evidence-based assessment and treatment, in line with the patient's wishes and in conjunction with the responsible Consultant. Provide support and development to junior team members and students. Provide support to B6 peers and Clinical Lead when appropriate. Provide quality triage and advice to patients, peers, other Community Teams, and Health Professionals when required.
