Job Overview Come and join our CQC rated ‘Outstanding’ Trust as a Community Nursing Sister /Charge Nurse for the Windsor and Maidenhead Community Nursing Team.

As a Community Nursing Sister /Charge Nurse, you will manage a clinical caseload under the delegation of the team leader, as well as exercising leadership and management of a defined community DN team to ensure good clinical practices and high standards of care are maintained in order to meet service demand. Working within our Community Nursing Team, you will support, assess, and treat patients with a variety of health needs, delivering high-quality, evidence-based practice. You will have a passion and enthusiasm to care for patients in their own homes.

Main Duties of the Job

Maintain required competencies, as identified in the Band 6 Community Nurse competency framework, in order to effectively undertake patient assessment, care planning, and immediate care needs.

Support staff to complete holistic assessment and reassessment of all health and nursing needs, ensuring that every patient has an overview of health and nursing need recorded.

Manage and be responsible for a clinical caseload under the delegation of the team leader.

Support teams to provide information to patients, their families, and carers both formally and informally, so that they can make informed choices about current and future care needs.

Provide clinical support, leadership, and guidance to all staff to ensure that evidence-based care and clinical practice are maintained in Community Nursing Teams.

Hold a full UK Driving Licence and have use of a car for work purposes.

Registered Nurse - Adult

Active on the mentor register

Continuous Professional Development

Evidence of continuing professional development

Previous Experience

Experience of leadership within a team

Wound care experience including leg ulcers

Experience at band 5 in a community setting or other relevant practice area

Knowledge, Skills & Abilities

Understanding of Community Nursing

Ability to work autonomously

Ability to support staff to undertake clinical duties, develop and implement evidence-based practice

Good organisational skills

IT, written, and oral skills including presentations

In-depth knowledge of leg ulcers aetiology, treatment, and prevention

Additional Requirements

Valid full UK licence with confidence to drive during social hours and has use of a car for work purposes
