Surrey Downs Health & Care We are seeking an enthusiastic and proactive individual with experience working alongside therapists or supporting individuals with stroke and neurological conditions. Strong communication skills and the ability to assist patients with activities of daily living are essential. You will receive professional support for your Continued Professional Development from a qualified therapist with expertise in neurological care. Support neuro-rehabilitation by implementing therapy programmes devised by the lead therapist. Manage your own patient caseload, assessing and treating patients, and monitoring their progress. Assist in patient assessments, gathering relevant information, and adapting treatment plans under therapist guidance. Work closely with the multidisciplinary team, contributing to patient care planning and discharge processes. Surrey Downs Health and Care deliver care closer to people’s own communities through our Primary Care Networks, Community Hospitals, Specialist Services and our innovative partnership of local NHS organisations. Surrey Downs Health and Care has a track record of providing person-centered care that goes beyond organisational boundaries to do what is best for the individual.
