Band 6 Senior Clinical Assessor - SPA Full-time - 37.5 hours per week Bromley Healthcare has a single point of clinical access within its Care Coordination Centre. Core to this operation is a team of Clinical Assessors made up of both nurses and therapists, who will triage all referrals coming into the organisation and then direct to the most appropriate clinician, using the organisation's resources as efficiently as possible with the aim of enabling as many people as possible to be cared for within their own homes. Main duties of the job

Act as the point of clinical contact for all referrals to Bromley Healthcare, including the triage and prioritisation of referrals and the provision of specialist advice to both referrers and internal clinicians. Manage referrals into the appropriate clinical team, demonstrating sound clinical decision making and effective use of EMIS and the Electronic Patient Record (EPR) systems. Manage the safe transfer of care between care providers and make appropriate referrals to other care providers outside of Bromley Healthcare. Make consistent judgements requiring complex fact analysis, interpretation and comparison of options, ensuring clinical reasoning is reflected accurately in case records. Balance clinical risk and decision making against context, ensuring patients receive safe, effective and timely care. Utilise allied health care professional expertise and clinical skills to enhance the experience of patients under our care. Enable early supported discharge, prevent inappropriate re-admission to hospital, share nursing knowledge and skills, and support technological advances at Bromley Healthcare. Support and participate in the development of integrated pathways and care packages for safe patient care outside of the acute hospital setting. Work closely with multi-disciplinary agencies and Primary Care professionals to provide appropriate and seamless care for patients and their families in the community. Provide clinical leadership and management of the Care Coordination Centre, continuously monitoring and maintaining clinical standards and quality of care. Monitor effectiveness of BHC protocols and triage requirements, escalating level of care for acutely ill patients when necessary. Qualifications

Current HCPC registration Registered with a statutory professional body with significant post qualification experience Significant Acute or Community Care experience Evidence of Continuing Professional Development Experience in Triaging of patients End of Life course Additional courses undertaken at Band 6 level Experience

Clinical knowledge in the care and management of the acutely ill / complex care / palliative care / intermediate care patient within community Knowledge of risk management Ability to work with and motivate nurses/therapy and medical staff Computer literate – confident in using a range of IT platforms Understanding of and ability to implement evidence-based practice Experience in managing and delivering sustained change Specific Skills

Excellent written, verbal and electronic communication skills Willing to learn new technologies related to the role Specialist and extensive expertise across a diverse range of professions under the title Registered Nurse/Allied Health Professions Evidence of experience in discharge planning and discharge pathways An interest in developing further clinical skills Supervising junior members of staff.
