We are currently looking to recruit an enthusiastic and motivated senior community nurse to join our team. The successful candidate will be based at Pershore Community Hospital and will provide leadership and support to their colleagues working within the Pershore and Upton Neighbourhood Team. We are looking for a passionate individual who is excited by change and is keen to deliver nursing care in the community. The successful post holder will be triaging calls that are coming into the Neighbourhood Team. You will be able to achieve a balance of triaging and patient facing visits, as you will also be visiting patients within their home environments, conducting holistic assessments for our Intensive Support patients who are coming out of hospital via Pathway 1, or completing an Admission Prevention assessments. We are a supportive team who are committed to developing services to provide individual care for our patients. We offer dedicated time for training and clinical supervision, with a supportive L&D Department to help guide. We will provide you with resources to develop and progress, as well as flexibility, professional development and a wide range of wellbeing and support. There is an expectation for the successful post holder to work weekends and late shifts, to ensure the service is covered to provide safe nursing care. Main duties of the job

The role will require: Working flexibly to provide the service delivery need across the local area (the service operates from 7:30am 6:30pm, Monday to Sunday including bank holidays). An enthusiastic, motivated and innovative thinker to become part of a primary care facing community. The ability to work autonomously and manage your own workload and work within a multi-disciplinary team. Experience of working in a fast-paced and varied environment. Receiving calls for district nursing, occupational therapy and physiotherapy patients. You will use your clinical knowledge to triage these phone calls which may either result in a same day visit, or advice and guidance can be given to promote self-care and independence. You will work alongside administrators who will also be receiving phone calls, and as the triaging clinician you will provide support and guidance to them also. You will manage and oversee the Intensive Support caseload, undertaking nursing assessments and reviews (72 hours and 1 week), to maintain your own holistic assessment skills. You will be involved in team meetings, goal/care planning and evaluation and handover meetings. You will also support the teaching and learning of nurse students pre and post registration. Work as an autonomous practitioner with the integrated care team providing rapid and planned expert professional assessment and treatment interventions to patients in their own homes or other community settings to enable patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible. This will be achieved through a combination of hands-on expert clinical skills and knowledge to treat, educate and support patients on a designated caseload and by working with patients and their carers to assess, plan and implement appropriate packages of care. Demonstrate expert clinical knowledge, health promotion and holistic assessment skills and will work with the patient and their carers to assess, plan, implement and evaluate their ongoing care and support needs in their own home or temporary/usual place of residence. Support the Operational and Clinical Team leaders in the delivery of best nursing practice, undertake triage, and an involvement with the allocation of work and leadership of other staff, manage team resources and deputise in their absence. Qualifications

Registered Nurse (RN1) Graduate Nursing Qualification at Academic Level 6 or working towards. Preceptorship training. Evidence of ongoing continuing professional development. I understand that I will be asked to provide original certificates to verify my qualifications at point of offer. Prescribing Qualification or willingness to undertake. Post-graduate modules in Leadership. Practice Assessor. Experience

Post-registration experience of working in a community nursing setting. Experience of caseload management and/or organisation of staff. Experience of active participation in interdisciplinary team working. Skills & Abilities

Ability to communicate effectively and clearly in English in both written and verbal formats. Knowledge

High level of clinical knowledge and skill. Clinical Governance. Good knowledge of community services and care pathways. Health Improvement. Good working knowledge of positive risk taking and effective risk management strategies. Disclosure and Barring Service Check
