This post holder will report into an integrated leadership structure and provide senior operational support to the Associate Director for Bedfordshire Community Health Services; Nursing and Lead Nurse (ELFT) and Deputy Director of Unplanned Care (CCS). The post holder will provide operational support management and leadership to Primary Care at Home teams nursing services, as defined by the Associate Director for Bedfordshire Community Health Services; Nursing and Lead Nurse. The post holder will support the development of Neighbourhood Working and the implementation of the Fuller recommendations at place in Central Bedfordshire. In addition, this post holder will be responsible for the development, implementation and operational management of the Urgent Care Response services and the Virtual Ward pathway across Bedfordshire. The post holder will support the Deputy Director of Unplanned Care (CCS) to deliver opportunities to align and integrate Urgent Care Response and Virtual Ward services across Luton and Bedfordshire. Main duties of the job

To provide day-to-day operational support to nursing services, working with service managers to ensure smooth running of services. There will be a focus on quality, service user experience and monitoring and improving outcomes. To support the Deputy Director of Unplanned Care Services (CCS) in the development and implementation of the Urgent Care Response Pathways and Virtual Ward and provide operational management of those services. Establish and maintain the effective management of community services supporting the Associate Director for Bedfordshire Community Health Services; Nursing and Lead Nurse. Act as a role model for nurses, maintaining a visible and credible profile within community services, and as part of the wider Trust nursing leadership structure. To lead service development projects to improve patient care and deliver trust objectives for Community Services. To provide leadership for key nursing measures within Community Services. Provide clinical and professional leadership, support and advice to staff within Community Services on professional nursing issues. Work with System partners to implement the Urgent Care response pathway and Virtual ward across Bedfordshire. Support the development of neighbourhood working and the implementation of the Fuller Recommendations at place in Central Bedfordshire. Provide Operational management to Community Nursing teams across the Central Bedfordshire council area. Person Specification

Education

Educated to Master's degree level or equivalent. Currently Registered with the NMC. Experience

Significant experience of managing at least one Community Service i.e. Specialist Nursing service, Primary care at Home Team services Lead/Head of Service, ideally across more than one community discipline and in more than one setting i.e. acute and community /Local authority services. Experience of working at a senior level, and in conjunction with external stakeholders e.g. partners in primary care and local commissioners. Knowledge and Skills

Significant clinical knowledge and understanding in own area. Ability to communicate complex ideas and concepts and adapt communication style according to audience. Ability to write clear and concise reports for discussion at senior level. Ability to analyse data and interpret significance of data and apply this to service development across all disciplines. Ability to problem solve and implement solutions. Ability to navigate difficult situations and find an outcome even when this isn't obvious. Other

Ability to use computer and all relevant applications. Ability to drive across county when required (hold a driving license). Disclosure and Barring Service Check
