KCHFT are seeking ambitious individuals to join our Community Assessment Bed (CAB) team. CAB support some of the most complex transfers of care from hospitals mainly in East Kent to the community. This is a critical administration role in the management of community assessment bed occupancy in East Kent. Your role will require you to demonstrate the Trust's CARE values and utilise these values on a daily basis when liaising with families, other Organisations or our providers. Attention to detail is key in this role as we support patients, families and next of kin's through the Discharge to Assess pathway. This can be a very anxiety provoking time for all involved so clear, consistent communication is key. Main duties of the job
Working in a high pressure service, you will report to the Purchasing and Allocation Coordinator and be responsible for ensuring referrals are processed, appropriate community beds are identified, effective communication facilitated between wards and internal/external providers and to ensure the safe and timely transfer of patients from acute hospitals. You will be responsible for keeping accurate, 'real time' records of all patient transfers and supporting the team in ensuring all processes are followed, and patients and their families are supported. Qualifications
Diploma of higher education, Foundation degree, Higher national diploma (HND), NVQ level 5 or equivalent Certificate in management or equivalent level 4 qualification Experience
Proven experience in office systems, with advanced use of Microsoft Office. Sound judgement and logical approach to complex situations, with ability to determine solutions. Practical application of HR practices, policies, and procedures. Basic experience of a financial system. Leading teams in busy and pressurised environments. NHS specific IT system experience.
