Crays Primary Care Network are looking for an enthusiastic, forward-thinking Clinical Pharmacist to join their ever-growing team. This position has become available due to the expansion of our existing team that has been growing with significant positive impact upon patient services across Crays PCN. The Crays Collaborative PCN is a network of 6 GP surgeries which are Broomwood Surgery, Crescent Surgery, Derry Downs Surgery, Gillmans Road Surgery, Poverest Medical Centre and St Mary Cray Practice. Together they cover a patient population of over 35,500 patients, most of whom live in deprivation. Two of their practices are training practices. They are a highly supportive PCN whose practices work closely together, understanding the importance of a good work/life balance with an open-door ethos. The PCN caters to the healthcare needs of their patients by ensuring they make the best use of collective resources across practices, local healthcare providers and professional groups to allow greater resilience to primary care. To achieve this, they are supported by additional roles such as paramedics, Clinical Pharmacists, social prescribers, Health and wellbeing coach, nurses, care coordinators, and mental health practitioners. You can expect to work across no more than 2-3 practices across the PCN. Main duties of the job

Clinical Pharmacists play a key role in Primary Care Networks. Their responsibilities include: Meds optimisation within the PCN Medicines Reconciliation Providing expert advice on clinical medicines Undertaking structured clinical reviews with patients to manage complex polypharmacy needs Working as part of a Multi-Disciplinary Team (MDT) in a patient-facing role to clinically assess and treat patients. Job responsibilities

Provide professional advice and support on the safe prescribing, handling and administration of medicines, including support regarding development and implementation of medicines management processes and policies. Lead on the medicines aspects relating to the care of patients to optimise medicines, reduce medicines-related risks and improve adherence to taking medicines by carrying out medication assessments and reviews in identified patients. Resolve day-to-day medicines and prescription issues with healthcare professionals. Help patients to manage their conditions. Lead repeat prescription services and deal with queries from staff and patients. Present to Patients any changes to prescribing in line with NICE, best practice or in response to prescribing alerts. Advise patients of the adverse side effects of medication and potential interactions with other medication/treatments. Rationalise repeat prescriptions lists to avoid waste and duplication. Provide reviews to those patients with LTCs or complex health needs, including drug monitoring and risk versus benefit. Provide both face-to-face and digital appointments to practice patients. Contribute to the management of the prescribing budget through the promotion of safe, cost-effective rational prescribing. Contribute to minimising the harms associated with the use of medicines, for example by deprescribing. Person Specification

Qualifications

Registered with the General Pharmaceutical Council. Postgraduate qualification in clinical/community pharmacy or equivalent additional experience. Evidence of recent continuing learning/professional development. Diploma in clinical/community pharmacy. Specific Skills

Excellent interpersonal skills. Ability to work on own initiative, able to plan, prioritise, make decisions and meet deadlines. Ability to work well within a team. Good organisational skills. Good time management. Ability to deal with difficult situations with diplomacy and sensitivity. Confident and assertive in dealing with other healthcare professionals. Knowledge of GP computer systems, especially EMIS. Familiarity with analysing/interpreting ePACT data. Knowledge

Experience of working in multi-disciplinary teams. Experience of medication review. Proven clinical pharmacy knowledge and evidence of commitment to continuing education. Understanding of primary and secondary care interface. Understanding of current medicines management issues. Knowledge of the changing NHS environment and structures. Familiarity with the concepts of evidence-based medicine, needs assessment, effectiveness, prioritisation and appropriateness. An appreciation of the complexities of relationships across the health economy. Experience of working in Primary Care. Personal Qualities

Excellent interpersonal skills particularly with prescribers, older people and carers. Ability to deal with difficult issues/situations with diplomacy and sensitivity. Ability to work on own initiative, able to plan, prioritise, make decisions and meet deadlines. Ability to work well in a team. Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £46,000 to £52,000 a year depending on experience.
