Pathway 3 supports discharge to local care home for individuals who require a further period of assessment to determine the long-term plan of care when an acute hospital admission is no longer required.
We work in collaboration with the acute trust Discharge Liaison Teams, Kent County Council Social Services Teams, All Age Continuing Health Care, local community services and organisations including the voluntary sector.
Main duties of the job In this role, you will be visiting patients in their homes and assessing their needs to support the development of a long-term care plan.
The community-based nature does mean that a driving licence with access to a car are essential.
Communication and compassion are fundamental to the role; liaising with partner organisations, patients, relatives, and carers is vital to ensure processes are understood and supported by all involved in the individual's care and assessment.
