Are you an experienced Children's Nurse looking for a new challenge? We are looking to recruit to our Children’s Community Nursing service where you will join an established supportive team here in Cornwall.

Due to service expansion and a future move to a 7 day working pattern, we are excited to have a Band 6 Children's Community Nurse for Transition vacancy available. This role is county wide and focuses on supporting the coordination of the transition of children and young people from the Children’s Community Nursing Services into adult services, as well as assisting with complex discharge from acute care into the community setting while continuing to use your nursing and caseload management skills for the care of children in the community.

Initially, due to Maternity Leave, the role will focus on holding a CCN caseload while scoping the transition processes. Following this period, the role will move to support the transition for children and young people while continuing to hold a clinical caseload.

This is a permanent contract for 37.5 hours currently divided over 4 days per week, to meet service needs. This role includes weekend and bank holiday working and occasional on-call requirements for End of Life care.

You will have current experience in looking after acutely unwell children. You must be highly motivated, possess excellent acute clinical assessment skills, and be able to communicate and organize effectively. You should be experienced in working under pressure and have the ability to work independently.

We require your skills and enthusiasm to develop our strategy in supporting unnecessary hospital admissions and reducing length of stay by facilitating early and safe discharge from the acute trust, as well as supporting children and young people with long-term or palliative conditions. You will hold your own caseload, taking responsibility for planning, coordinating, delivering, and evaluating care given.
