We are currently seeking a dedicated, caring, compassionate & highly motivated Senior Community Nurse to join our team. Applications are welcomed from nurses who have 2 or more years' experience post qualification and previous roles working as a community nurse will be advantageous.

Main duties of the job The Community Nursing Service aims to provide an excellent standard of person-centred planned and unplanned nursing intervention to patients in their homes and is part of an integrated service that supports patients to remain in their home environment. As a County Wide Community Nurse you will work closely with the multi-disciplinary team including GPs, Practice Nurses and Specialist Nurses in order to support the service delivery having a high focus on quality outcomes and improving the patient experience. As both an organisation and a service we are actively involved in both local and national innovative projects around improving quality of care and integrated working. We promote mobile working technology to aid staff to provide a safe robust service for the patients. We have an extensive program of training and skills development available and actively nominate and encourage our staff to apply for national awards, such as Queen's Nursing Institute, RCN and Nursing Times awards. Motivated staff are encouraged to complete extended skills training such as the DNSPQ. The Trust also supports unqualified team members with appropriate experience to complete Nurse Associate training and for those who can demonstrate capability, Registered Nurse training.

About us NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
