Candidates with no experience will start at c £39,000 My client is the leading provider of Case Management services in England, and they are seeking a self-directed, autonomous practitioner to join their UK wide team. You will case manage a group of 30+ patients who are based throughout your territory. The territory you cover can be up to 2 hours from your home. Day to Day You will initially assess and then manage the case of a wide range of patients. Typically, you will work with a case over 18 months. The patient groups will have complex orthopaedic injuries, spinal injuries and acquired brain injury. You will be expected to manage your caseload remotely and when required reassess the patient. Lead the case management for individuals who have sustained complex physical or psychological injuries. Assess the individual’s needs to develop and manage a treatment plan and overall return to function and work. Work closely with the individual and their employer to achieve positive outcomes. Write findings and recommendations in reports to stakeholders. Management of a financial delegation per case. As the position is field-based, there will be a requirement for travel and the occasional overnight stay. Requirements No experience in case management is required, however, a background in complex orthopaedics is crucial. Interest in case management and demonstrating you can commit to the role and the patient group. Those candidates with case management experience gained in the Personal Injury sector are eagerly sought. Training You will receive an intensive training course which typically lasts 6 months. You will have 1-2-1 support from your manager and the opportunity to develop further within the role.
