Support Worker, Derbyshire County Council, Adult Community Care Team - Must have a minimum of 2 years+ UK experience within an Adult Social Work team. Our client is looking for a Support Worker with 2 years+ UK experience within an Adults team. The responsibilities will include: Arranging and Supervising of contact Assisting in the assessment process Commissioning services both internally and externally Undertaking complete holistic assessments of service user needs on less complex cases, to include a range of outcomes Arranging/providing transport for/or acting as escort to clients Disseminating information to clients (e.g., facilities available in the community and/or provided by the Department) Assisting Social Workers and others in the implementation of assessment, intervention, and care plans Liaison with voluntary bodies and other services, agencies, or groups working in the area, as appropriate Serving on ad hoc bodies/working parties, etc. Recording of all work undertaken and the preparation of reports/assessments Assisting or running group and community projects in the area, as appropriate Undertaking required training Maintaining confidentiality and observing data protection guidelines Carrying out any other reasonable duties within the overall function, commensurate with the grading and level of responsibility of the post Providing services to all client groups if required, including the more routine statutory visiting of children in care Involving, jointly with the Service Manager, in reviewing work and caseload at regular intervals, including the closure and transfer of cases Undertaking reviews of service users and their care plans The successful candidate will need to hold an NVQ3 in Health & Social Care or equivalent, have a current DBS, and be a car driver.
